ABSTRACT

Sleep staging is the basis for solving sleep problems. There's an upper limit for the classification accuracy of sleep staging models based on single-channel electroencephalogram (EEG) data and features. To address this problem, this paper proposed an automatic sleep staging model that mixes deep convolutional neural network (DCNN) and bi-directional long short-term memory network (BiLSTM). The model used DCNN to automatically learn the time-frequency domain features of EEG signals, and used BiLSTM to extract the temporal features between the data, fully exploiting the feature information contained in the data to improve the accuracy of automatic sleep staging. At the same time, noise reduction techniques and adaptive synthetic sampling were used to reduce the impact of signal noise and unbalanced data sets on model performance. In this paper, experiments were conducted using the Sleep-European Data Format Database Expanded and the Shanghai Mental Health Center Sleep Database, and achieved an overall accuracy rate of 86.9% and 88.9% respectively. When compared with the basic network model, all the experimental results outperformed the basic network, further demonstrating the validity of this paper's model, which can provide a reference for the construction of a home sleep monitoring system based on single-channel EEG signals.

ABSTRACT

Polysomnography (PSG) monitoring is an important method for clinical diagnosis of diseases such as insomnia, apnea and so on. In order to solve the problem of time-consuming and energy-consuming sleep stage staging of sleep disorder patients using manual frame-by-frame visual judgment PSG, this study proposed a deep learning algorithm model combining convolutional neural networks (CNN) and bidirectional gate recurrent neural networks (Bi GRU). A dynamic sparse self-attention mechanism was designed to solve the problem that gated recurrent neural networks (GRU) is difficult to obtain accurate vector representation of long-distance information. This study collected 143 overnight PSG data of patients from Shanghai Mental Health Center with sleep disorders, which were combined with 153 overnight PSG data of patients from the open-source dataset, and selected 9 electrophysiological channel signals including 6 electroencephalogram (EEG) signal channels, 2 electrooculogram (EOG) signal channels and a single mandibular electromyogram (EMG) signal channel. These data were used for model training, testing and evaluation. After cross validation, the accuracy was (84.0±2.0)%, and Cohen's kappa value was 0.77±0.50. It showed better performance than the Cohen's kappa value of physician score of 0.75±0.11. The experimental results show that the algorithm model in this paper has a high staging effect in different populations and is widely applicable. It is of great significance to assist clinicians in rapid and large-scale PSG sleep automatic staging.

ABSTRACT

Objective:To investigate the predictive value of Charlson comorbidity index (CCI) in the operative prognosis of colorectal cancer (CRC).Methods:The retrospective cohort study was conducted. The clinicopathological data of 1 337 CRC patients who underwent surgery in the First Affiliated Hospital of Xi'an Jiaotong University from January 2013 to February 2019 were collected. There were 774 males and 563 females, aged 62(range, 22?80)years. All patients were evaluated by CCI. Observation indicators: (1) clinicopathological characteristics of CRC patients undergoing operation; (2) follow-up and survival; (3) prognostic factors analysis of CRC patients undergoing operation; (4) establishment and evaluation of a nomogram prediction model based on CCI. Follow-up was conducted using the telephone interview or outpatient examination to detect the survival of patients up to March 2020. Measurement data with normal distribution were represented as Mean± SD, and comparison between groups was analyzed using the t test. Measurement data with skewed distribution were described as M(range) or M( Q1, Q3), and comparison between groups was analyzed using the Mann-Whitney U test. Count data were described as absolute numbers or percentages, and comparison between groups was analyzed using the chi-square test. Non-para-meter rank sum test was used for comparison of ordinal data. The Kaplan‐Meier method was used to calculate survival rates and draw survival curves, and Log-Rank test was used for survival analysis. Univariate and multivariate analyses were performed using the COX proportional hazard regression model. The independent risk factors were included into R4.0.4 software to construct a nomogram prediction model. The receiver operating characteristic (ROC) curve was drawn, and the area under curve (AUC) was used to evaluate discrimination of the nomogram prediction model. The C-index and calibration chart were used to evaluate consistency of the nomogram prediction model. Results:(1) Clinicopathological characteristics of CRC patients undergoing operation. Of the 1 337 patients, there were 1 041 cases with CCI ≤3 and 296 cases with CCI ≥4. Age, cases with non-smoking history, smoking cessation or smoking history, cases without or with R 0 resection, cases with low, moderate, well differentiated tumor, cases in stage Ⅰ?Ⅱ or Ⅲ?Ⅳ of clinical TNM staging, preoperative carcinoembryonic antigen (CEA) were 61(53,68)years, 717, 43, 281, 12, 1 029, 123, 859, 59, 666, 375, 3.22(1.84,7.75)μg/L for the 1 041 patients with CCI ≤3, versus 70(61,75)years, 217, 19, 60, 43, 253, 48, 237, 11, 102, 194, 5.55(2.43,17.64)μg/L for the 296 patients with CCI ≥4, showing significant differences in the above indicators between them ( Z=?10.50, χ2=7.34, 104.51, Z=?2.31, χ2=82.14, Z=?5.78, P<0.05). (2) Follow-up and survival. All the 1 337 patients were followed up for 31(range, 1?84)months. Of the 1 337 patients, 1 024 cases survived and 313 cases died. The 1-, 3-, 5-year survival rates were 94.8%, 85.5%, 80.1% for the 1 041 patients with CCI ≤3, versus 73.6%, 46.9%, 34.0% for the 296 patients with CCI ≥4, showing significant differences between them ( χ2=181.93, P<0.05). (3) Prognostic factors analysis of CRC patients undergoing operation. Results of univariate analysis showed that age, smoking history (having a history of smoking), tumor location (decending colon-sigmoid colon, recto-sigmoid junction-rectum), R 0 resection, tumor differentiation degree (moderate differentiation, well differentiation), clinical TNM staging, postoperative radio-therapy and chemotherapy, preoperational CEA and CCI were related factors for operative prognosis of CRC patients ( odds ratios=1.76, 0.71, 0.72, 0.61, 0.08, 0.39, 0.13, 3.02, 0.60, 2.41, 4.96, 95% confidence intervals as 1.39?2.23, 0.53?0.93, 0.52?0.99, 0.47?0.78, 0.06?0.11, 0.30?0.50, 0.05?0.31, 2.39?3.81, 0.48?0.76, 1.92?3.01, 3.97?6.20, P<0.05). Results of multivariate analysis showed that age >60 years, clinical TNM staging as stage Ⅲ?Ⅳ, preoperational CEA >5 μg/L and CCI ≥4 were independent risk factors for operative prognosis of CRC patients ( odds ratios=1.29, 1.88, 1.77, 2.84, 95% confidence intervals as 1.00?1.65, 1.45?2.44, 1.40?2.23, 2.20?3.67, P<0.05);tumor located in descending colon to sigmoid colon and recto-sigmoid junction to rectum, R 0 resection,tumor differen-tiation degree as moderate and well differentiation, postoperative radiotherapy and chemotherapy were independent protect factors for operative prognosis of CRC patients ( odds ratios=0.71, 0.72, 0.27, 0.50, 0.25, 0.56, 95% confidence intervals as 0.51?0.98, 0.56?0.93, 0.19?0.37, 0.38?0.65, 0.10?0.62, 0.44?0.70, P<0.05) (4) Establishment and evaluation of a nomogram prediction model based on CCI. Based on age, tumor location, R 0 resection, tumor differentiation degree, clinical TNM staging, postoperative radiotherapy and chemotherapy, preoperational CEA and CCI of multivariate analysis results, a nomogram prediction model for operative prognosis of CRC patients was established. The nomogram score was 1.0 for age >60 years, 18.0 for tumor located in proximal colon, 9.0 for tumor located in distal colon, 53.0 for non-R 0 resection, 62.0 for low differentiated tumor, 31.0 for morderate differentiated tumor, 32.0 for stage Ⅲ?Ⅳ of clinical TNM staging, 26.0 for no postoperative radiotherapy and chemotherapy, 4.6 for each increase of 100 μg/L in preoperative CEA and 12.6 for each increase of 1 score in CCI respectively. The total of different scores for risk factors was used to evaluate total 1, 3, 5-year survival rates. The ROC curve was drawn to evaluate the predictive ability for prognosis of nomogram model, with the AUC as 0.75 (95% confidence interval as 0.71?0.79, P<0.05). The C-index was 0.80 (95% confidence interval as 0.77?0.72). The calibration chart showed a good consistency between the probability of survival predicted by nomogram and the actual probability of survival. Conclusions:Age >60 years, stage Ⅲ?Ⅳ of clinical TNM staging, preoperational CEA >5 μg/L and CCI ≥4 are independent risk factors for operative prognosis of CRC patients. Tumor located in descending colon to sigmoid colon and recto-sigmoid junction to rectum, R 0 resection, tumor differentiation degree as moderate and well differentiation, postoperative radiotherapy and chemotherapy are independent protective factors for operative prognosis of CRC patients. The nomogram prediction model contributes to prediction of the survival of CRC patients.

ABSTRACT

Objective To investigate the influence from natural and constrained modal of the hip joint on internal fixation after implantation of hollow screws and locking plates for treating femoral neck fractures. Methods CT image data of a patient with femoral neck fracture were analyzed, boundary of the hip joint was extracted to generate a three-dimensional (3D) model of the hip joint, and the assembly of common internal fixation models and hip joint models was established. Finite element simulation was then conducted, with focus on vibration characteristics. Results Vibration had a certain effect on internal fixation stability of hollow screws and locking plates. The occurrence of torsion would destroy the fixation environment of hollow screws and locking plates, resulting in a small displacement at the fracture end. In a constrained state, the modal frequency range was lower. Deformation of the vibration shape mostly occurred at proximal end of the femoral head, leading to the loosening phenomenon of internal fixation and prosthesis. A method for judging the stability of internal fixation was proposed from the perspective of vibration characteristics, and it was found that the stability of internal fixation with hollow screws was better than that of proximal locking plates. Conclusions For choosing internal fixation, influences from natural frequency of the screw and plate should be considered, so as to avoid natural frequency of the femur, which may cause resonance. The results can be used as a guidance for the selection of internal fixation materials and the design of structure and configuration.

ABSTRACT

Objective To analyze the biomechanical feasibility of two-point fixation by distal radius plate for the treatment of SandersⅢ calcaneal fractures. Methods The three-dimensional (3D) finite element musculoskeletal foot model was established based on CT and MRI images, which comprised bones, muscles, plantar fascia, ligaments and soft tissues. After validation, the SandersⅢ calcaneal fracture models fixed by distal radial plate (two-point fixation) and calcaneal plate (three-point fixation) were established, so as to compare the biomechanical characteristics of two calcaneal models. Results The maximum stress of the two-point fixation and three-point fixation model was 324.70 and 407.90 MPa, respectively. The maximum displacements of the two models were 2.498 and 2.541 mm, respectively. There was no significant difference in the posterior articular surface displacement between the two models. In both models, the Bohler’s angle and Gissane’s angle were within the normal range. Conclusions The two-point fixation by distal radial plate can satisfy the biomechanical stability of calcaneal fracture treatment. Compared with traditional steel plate, the two-point fixation shows the advantage of smaller surgical trauma, more uniform overall stress distribution, early weight-bearing rehabilitation after surgery, which is a novel treatment recommended for treating calcaneal fractures.

The system is used to realize the rational deployment suggestions and evaluation of medical equipment in hospitals. The system input the data of hospital medical equipment and clinical pathway into the database, and then feedback the deployment information to users which are calculated by big data information, so as to achieve the purpose of giving rational deployment of hospital medical equipment.

ABSTRACT

Calcaneal fractures, common injury to foot, usually require surgery if complicated with an obvious displacement. Since open reduction and internal plate fixation often leads to wound infection, more and more minimally invasive surgery has been adopted for calcaneal fractures. As a minimal invasive treatment, intramedullary fixation can be used to treat calcaneal fractures of Sanders types Ⅰ-Ⅳ. Calcanail and C-Nail are 2 major types of intramedullary nails that have been used to treat calcaneal fractures in Europe and the United States since 2011. This article reviews the basic structural features, biomechanical properties, surgical procedures and clinical efficacy of these intramedullary nails.

ABSTRACT

Nano-metallic materials are playing an important role in the application of medicine, catalysis, antibacterial and anti-toxin due to their obvious advantages, including nanocrystalline strengthening effect, high photo-absorptivity, high surface energy and single magnetic region performance. In recent years, with the increasing consumption of global petrochemical resources and the aggravation of environmental pollution, nanomaterials based on bio-based molecules have aroused great concern. Bio-based molecules refer to small molecules and macromolecules directly or indirectly derived from biomass. They usually have good biocompatibility, low toxicity, degradability, wide source and low price. Besides, most bio-based molecules have unique physical, chemical properties and physiological activity, such as optical activity, acid/alkali amphoteric property, hydrophilic property and easy coordination with metal ions. Thus, the corresponding nano-materials based on bio-based molecules also have unique functions, such as anti-inflammatory, anti-cancer, anti-oxidation, antiviral fall blood sugar and blood fat etc. In this paper, we give a comprehensive overview of the preparation and application of nano-metallic materials based on bio-based molecules in recent years.

ABSTRACT

Objective A three-dimensional (3D) printing precise pressure device was designed specifically targeted at cambered limbs according to the requirement of postoperative rehabilitation of total knee replacement(TKR), and its effectiveness and safety was verified by finite element analysis. Methods Based on gastrocnemius muscle of lower limbs as the pressurized objects, the precise pressure device was designed, which contained an air pressure generating module, an inflatable airbag and a 3D printing brace. Through the closed loop control algorithm, the device stably supplied different pressures in the airbag. Distributed pressure data of the airbag-skin within contact surface were collected under different experimental conditions and imported into biomechanical simulation software which combined CT images to reconstruct 3D model of the lower limb mechanics. Finally, the effective compression area fraction and the joint micro-motion angle under each condition were obtained, to verify the effectiveness and safety of the system. Results Using generally preferred 4 cm-size offset and 4-barrel airbag configurations, under different intracapsular pressure of 5.32,6.65,7.98,9.31,10.64 kPa, the simulated knee joint micro-motion angles were 5.3°, 6.1°, 7.2°, 9.5°, 10.6°, respectively, and the effective compression area fraction could be up to 90-8%-95-2%. Conclusions For the optimized scheme, the dynamic range of joint micro-motion angle and the effective compression area fraction caused by different airbag pressure values were the best and met the design requirements of effectiveness and safety. The research findings can contribute to analyzing the influence of compression system on limb biomechanics, which are of great significance for effective and safe rehabilitation training after TKR.

ABSTRACT

Objective To study the effect from personalized insoles with different structural heel pads on plantar stress concentration of patients with heel pain. Methods Base on statistics and finite element analysis method, the finite element model of foot and personalized insoles for patient with heel pain were established. The effects from different insoles on stress of soft tissues and plantar fascia were simulated. Results The internal stress of plantar soft tissues was higher than that of plantar epidermis, and the stress of the third plantar fascia was the highest. During barefoot standing, the internal peak stress of plantar soft tissues was 1.34 times of plantar epidermis, and the stress of the third plantar fascia was 1.50 MPa. The result of orthogonal experiment showed that the optimized insole model could reduce the internal peak stress of plantar soft tissues by 51%, meanwhile relieve the stress of the third plantar fascia by 11.3%. Conclusions The optimum scheme of personalized buffer insole is the design of vertical ellipse and honeycomb groove. Such structure can assist the absorption or buffering of concussion from calcaneal fat pad, and relieve the plantar stress concentration and tension of the plantar fascia. This study is helpful to understand plantar stress distributions of patients with heel pain, which is of great significance to the study of pathology and prevention of heel pain.

ABSTRACT

Objective To investigate the mechanism of occurrence and rehabilitation of heel pain, so as to provide a theoretical basis for the effectiveness of heel pain treatment. Methods The CT and MRI data acquired from feet and knees of patients with heel pain were reconstructed to establish the bone-muscle composite finite element model. Based on the established model, the effect of calf muscle contracture on biomechanical properties of the foot and ankle was simulated by using the finite element method . Results When the calf muscles produce upward lifting power, plantar pressure was transferred from the heel area to the forefoot area, and there was no significant difference in pressure distribution by different combination schemes of muscle forces. The strain of the plantar fascia was increased, with stress concentration on the calcaneus surface. Under 240 N force, the peak stress at the Achilles tendon attachment position and the calcaneus nodules was up to 10.82 MPa and 11.2 MPa, respectively. Conclusions The stress concentration in calf muscles and Achilles led to the changes in biomechanics of the ankle, which resulted in heel pain. The method of improving the overall biomechanical environment by releasing concentrated stress to restore the position of the bones and joints of the ankle joint is the mechanism for rehabilitation of heel pain.

ABSTRACT

Objective: To explore the ethical cognition of scientific researchers on stem cell transplantation,and provide a basis for improving their ethical accomplishment.Methods: Adopting qualitative research method,this paper conducted the interview in depth among 15 researchers involved in the stem cell transplantation research aiming at ethical issues about stem cell transplantation and analyzed the interview texts.Results: The scientific re-searchers' ethical cognition on stem cell transplantation and macroscopic cognition on medical ethics were reflected in the following aspects: a certain degree of understanding on the ethical issues about stem cell transplantation; not attaching importance to ethical review in stem cell transplantation research; lack of ethics education; lack of under-standing on the conflict of interest for stem cell therapy; attaching importance to technology and despising ethics in macro concept.Conclusion: The ethical consciousness of Chinese researchers in the stem cells therapy is weak.It is recommended that it should strengthen the education of medical ethics and the ethical review of scientific research projects,so as to standardize the ethical procedures.

ABSTRACT

Objective To understand the situation of iodine deficiency disorders and iodine nutrition of children in Yunnan Province after the implementation of new standard of salt iodine,and provide the basic data for prevention and control of iodine deficiency disorders.Methods Using multistage sampling method,Yunnan provincial evaluations for iodine deficiency disorders elimination were carried out.According to the comparative analysis of 2010 (pre-adjustment) and 2015 (post-adjustment) evaluation results,the changes of the median of salt iodine,the coverage rate of iodized salt,the qualified rate of iodized salt,the rate of qualified iodized salt consumption,goiter rate of school children aged 8-10 and the median of urinary iodine were evaluated.Salt iodine was tested using the "Salt Industry General Test Method for Determination of Iodine" (GB/T 13025.7-1999);thyroid volume examination of children using B-scan ultrasonography,thyroid volume calculation and enlargement judgment using "Diagnostic Criteria for Endemic Goiter" (WS 276-2007);and urinary iodine detection using "Urinary Iodine Arsenic Cerium Catalytic Spectrophotometry" (WS/T 107-2006).Results The medians of salt iodine in 2010 and 2015 were 30.6 and 22.4 mg/kg,respectively,the difference was statistically significant (U =317 503.50,P < 0.01).The coverage rates of iodized salt in 2010 and 2015 were 99.6% (1 681/1 688) and 98.9% (2 592/2 622),the qualified rates of iodized salt were 98.6% (1 657/1 681) and 89.7% (2 325/2 592),and the rates of qualified iodized salt consumption were 98.2% (1 657/1 688) and 88.7% (2 325/2 622),and the differences were statistically significant (x2 =6.42,125.11,130.18,P < 0.01);the rates of children goiter in 2010 and 2015 were 0.8% (25/3 272) and 1.4%(44/3 245),respectively;the medians of urinary iodine of children were 289.0 and 157.3 μg/L,respectively,the difference was statistically significant (U =1 121 669.50,P < 0.01).Conclusion After decreasing of salt iodine level starting from 2012 in Yunnan Province,the children iodine nutrition condition is in a more proper level.

ABSTRACT

Multi-layer perceptron (MLP) neural network belongs to multi-layer feedforward neural network, and has the ability and characteristics of high intelligence. It can realize the complex nonlinear mapping by its own learning through the network. Bipolar disorder is a serious mental illness with high recurrence rate, high self-harm rate and high suicide rate. Most of the onset of the bipolar disorder starts with depressive episode, which can be easily misdiagnosed as unipolar depression and lead to a delayed treatment so as to influence the prognosis. The early identifica- tion of bipolar disorder is of great importance for patients with bipolar disorder. Due to the fact that the process of early identification of bipolar disorder is nonlinear, we in this paper discuss the MLP neural network application in early identification of bipolar disorder. This study covered 250 cases, including 143 cases with recurrent depression and 107 cases with bipolar disorder, and clinical features were statistically analyzed between the two groups. A total of 42 variables with significant differences were screened as the input variables of the neural network. Part of the samples were randomly selected as the learning sample, and the other as the test sample. By choosing different neu- ral network structures, all results of the identification of bipolar disorder were relatively good, which showed that MLP neural network could be used in the early identification of bipolar disorder.

ABSTRACT

The early damage detection and evaluation are of great significance in treatment and prognosis to the left ventricular function for children with tumor. In this paper, it is reported that the early damage of the left ventricular function was observed by pulsed wave Doppler (PWD) and tissue Doppler imaging (TDI) in our laboratory. Eighty children half a year to fourteen years old were included in this study. The cardiac function indices in chemotherapy group and control group were measured and compared. The results showed that there was significant difference in mitral and tricuspid annulus flow spectrum between the two groups. Compared with PWD,TDI is more prompt, objective and accurate in detecting early damage of left ventricular function in children with tumor. And TDI is a good method for early identification of ventricular function damage in children with tumor.

ABSTRACT

<p><b>OBJECTIVE</b>In order to understand the iodine nutritional status, after the salt-iodine content was showed a reduction in 2012 and to evaluate the current situation after the new standards was brought into force to the general population in an experimental community of Yunnan province.</p><p><b>METHODS</b>Randomly sampled urine and salt were collected, to test the iodine concentration in the study-site. Pre-and post-levels of the iodized salt under the provision of the new standards, were identified.</p><p><b>RESULTS</b>of this study were gathered upon 3 weeks or 3 months, respectively. Results Data from the three randomly chosen study sites showed that the urine iodine concentration in the general populations was reducing gradually. In the general population, medians of Urine Iodine (MUI) were 279.71 µg/L, 239.64 µg/L and 226.26 µg/L, respectively. Proportion of the urine iodine value for 100-199 µg/L increased but ≥300 µg/L decreased, after the new standard was put into practice. Both homogeneity and stability of the new standard on iodized salt seemed to be good.</p><p><b>CONCLUSION</b>Iodine nutrition in general population appeared reasonable under the use of newly set salt-iodine standards in general population living in Yunnan province.</p>

ABSTRACT

According to the working principle of vertical ultrasonic vial washing machine, receiving respective force of small water droplets on the inside wall of vials and the minimum air velocity of blowing off water droplets can be obtained based on the analysis of water-droplet-related parameters. The inside wall model of 7 mL vial created by GAMBIT was divided into fine grids. Then the Realizable k-epsilon Two Equation Turbulence Model was adopted and the flow field of vial by FLUENT software was simulated when air was flushing inside the wall. In that case, the optimal position, inner diameter and the corresponding minimum air velocity of needle can be acquired to meet the needs of vial washing machine applied to 7 mL vial.

ABSTRACT

Objective To study the effects of improved Wright-Giemsa staining. Methods 40 semen samples by both traditional and improved Wright-Giemsa staining.The morphological characteristics stained by two methods was ob-served. Results The defective rates of the middle and tail parts of sperm were 7.02%± 2.4%and 11.02%± 2.03%respec-tively in improved staining group;while those were 5.48%±2.8%and 8.05%±2.56%in traditional staining group. There was statistical significance between two staining methods (P < 0.05). There was no significant difference of normal sperm and sperm head between these two staining methods;however, improved staining method showed a much clearer nucleus and ac-rosome staining than that of traditional methods. Improved Wright-Giemsa staining is better for sperm, vaginal discharge and Trichomonas vaginalis. More importantly, axostyle and flagella were clearly stained with the improved method. Conclusion Improved Wright-Giemsa staining is simple and can be used in clinical diagnosis.

ABSTRACT

In order to realize tumor disease information sharing and unified management, we utilized grid technology to make the data and software resources which distributed in various medical institutions for effective integration so that we could make the heterogeneous resources consistent and interoperable in both semantics and syntax aspects. This article describes the tumor grid framework, the type of the service being packaged in Web Service Description Language (WSDL) and extensible markup language schemas definition (XSD), the client use the serialized document to operate the distributed resources. The service objects could be built by Unified Modeling Language (UML) as middle ware to create application programming interface. All of the grid resources are registered in the index and released in the form of Web Services based on Web Services Resource Framework (WSRF). Using the system we can build a multi-center, large sample and networking tumor disease resource sharing framework to improve the level of development in medical scientific research institutions and the patient's quality of life.
